Tile, grout, and caulk are surface finishes. They slow water down, but they don’t block it completely. Water moves through grout over time. It finds small gaps. It works its way into seams.
And once it gets behind the surface, there’s nothing stopping it unless a real waterproofing system is in place.

Is Tile Waterproof?
No. Tile is a surface finish, not a waterproofing material. Grout is porous and allows water to pass through over time. Caulk degrades and separates. Water moves through grout over time, finds small gaps, and works its way into seams. Without a dedicated waterproofing membrane behind these surfaces, water reaches the structure of your home.
What Causes Shower Waterproofing to Fail?
No waterproofing was ever installed.
Just cement board and tile. This is more common than most homeowners realize.
Partial waterproofing.
Some areas are covered, others aren’t. Corners, seams, or penetrations get skipped. That’s all it takes.
Poor integration with the drain.
